Emergency Department - RN Case Manager
12 hours shifts position available for a RN Case Manager in our Emergency Department. Is an active member of the entire ED team.
The role combines every function of case management, from access through discharge planning. Core responsibilities of the RN ED case manager are to provide consultation and directions for physicians about the appropriate determination of the level of care, specifically observation vs. inpatient status; to initiate discharge planning directly from the ED to levels such as acute rehabilitation, SNF and home care services; and to develop and implement case management plans for those patients who visit the ED frequently.
Applicants must be a RN graduate of NLNAC or CCNE School of nursing, possessing an Ohio license. BSN or 5 years of case management experience in an acute care setting.
