Position Summary
The incumbent for this position will provide Day Program activities and services. The goal of this role is to help individuals build natural supports, social networks, and experience meaningful community participation and inclusion. The community and day program services that this role implements are defined in Individual Service Plans (ISP).
Education: High School Diploma or General Equivalent Diploma.
Experience: One year of experience assisting individuals with Intellectual and Developmental Disabilities (IDD), building natural supports, social networking and participating in the community and/or day program facility.
Licenses, Certifications & Clearances:
- Act 33/34 Clearances
- FBI Clearance
- Valid State of PA Class 1 Driver’s License, reliable transportation, safe driving record, and ability to drive.
Responsibilities and Duties include:
- Instructs a monthly curriculum of adult meaningful activities for assigned individuals to participate in the community or at the facility. Encourages the participation of the activities, providing support and assistance to the individuals.
- Enhances integrated community supports and opportunities for individuals with disabilities. Explores and identifies interests, needs and wants of the individuals pertaining to community activities.
- Seeks out and builds effective relationships with a multitude of community organizations to enhance personal reliance, contribution and community connection for the individuals.
- Adheres to the guidelines of the Community Participation Supports service.
- Provides hands on direct support for personal hygiene (restroom), mealtimes and all other daily living skills as needed.
- Assists individuals in taking medications in accordance with Regulatory Authorities and Agency Policies/Procedures, with the Appropriate Training and/or Certification.
- Implements Individual Service Plans (ISPs) and other training programs for individuals. Maintains accurate documentation reflecting individuals’ progress on these programs via Credible, electronic record system.
- Utilizes appropriate behavioral and crisis intervention techniques in accordance with ISPs, Training Plans, Program Supervisor/Coordinator Direction, Regulatory Authorities, and Agency Policy/Procedures. Reports problems to and/or seeks assistance from the program Supervisor/Coordinator.
- Coordinates appropriate training activities in accordance with ISPs and the individual’s schedule.
- Observes individual’s basic conditions, such as medical and behavioral conditions of individual. Administers simple First Aid when necessary. Provides on-going emotional support and intervention to individuals by utilizing effective problem-solving techniques, the principles of positive approaches and current best practices in the field. Reports these conditions appropriately to program Supervisor/Coordinator.
- Assists in other programs as needed and performs other duties as assigned.
