Full-time position available with the Health Department, Children & Youth With Special Care Needs Division 2400 Troost.
As a condition of employment with the City of KCMO Health Department, all new employees will be required to provide documentation of having received an influenza (Flu) Vaccination.
Normal Work Days/Hours: Monday-Friday, 8:00 a.m.-5:00 p.m.
Provides service coordination for children and youth with special health care needs from birth to age 21. The program focuses on early identification and service coordination for individuals who have or are at increased risk for a medical condition that may hinder their physical growth and development. Provides program service coordination, which includes: Reviewing referrals and evaluating needs for resources. Gathers medical and financial information to determine program eligibility. Completes comprehensive assessments of needs for the participant and family. Sets goals with the participants/families and identify services needed to reach goals. Assists the participants/families to identify and communicate with their care team for support and advocate for services or equipment. Organizes and participates in team conferences with participants/families and other care team members. Reviews plan of care for ongoing needs and make changes as needed. Works with families to establish transition plans as the participants age out of the program, and as medical support needs change. Encourages participants/families to develop skills needed to function at their maximum level of independence. Provides outreach presentations and referral opportunities. Must be able to travel to the service regions covered by the CYSHCN program.
REQUIRES an accredited Bachelor’s degree and 2 years of health related work experience, including 1 years at a level comparable to a Public Health Specialist; OR an accredited Master’s degree and 1 year of health related work experience at a level comparable to a Public Health Specialist; OR an accredited Master’s degree in Public Health; OR an equivalent combination of qualifying education and experience. Must pass a preemployment drug screen and post offer physical examination as prescribed by the City.
Salary Range: $24.77-$37.16/hour.
